Premium Table: In the event that the insured is responsible for the deductible of the each treatment. (Under the inpatient benefit limit)
1. The insured is responsible for the deductible of 50,000 baht. Unit: Baht

2. The insured is responsible for the deductible of 100,000 baht. Unit: Baht

3. The insured is responsible for the deductible of 300,000 baht. Unit: Baht

Conditions:
1. Applicant has intention of applying for Foreigner in Thailand (O-A, O-X, Special Tourist Visa (STV))
2. All nationalities are eligible to apply for the O-A Visa, STV Visa and Foreigner.
3. For O-X Visa, applicants must hold passport and nationality of Japan, Australia, Denmark, Finland, France, Germany, Italy, Netherlands, Norway, Sweden, Switzerland, United Kingdom, Canada and United States of America.
4. Entry age 14 days up to 75 years old
5. Pre-existing conditions: No benefits provided for any illness or injury for which a Covered Person received any diagnosis, medical advice or treatment, or had taken any prescribed drug, or where distinct symptoms were evident prior to the effective date.
6. Waiting Period:
  6.1 No benefits provided for any sickness which occurs during the first 30 days from the first policy effective date, and
  6.2 No benefits provided for the following disease which occurs during the first 120 days from the first policy effective date:
    • Benign or malignant tumor or cancer or cystic mass
    • Tonsillectomy or adenoidectomy
    • Hemorrhoids
    • Stones
    • Hernias
    • Varicose Veins
    • Pterygium, pinguecula, cataract
    • Endometriosis
7. The Company shall provide coverage of treatment outside Thailand only for the Injury or the sickness as suddenly occurring which cannot be foreseen during being in foreign country to the extent of being treated as the inpatient.
8. Applicant must send the filled out and signed health application form together with a copy of passport for the company's consideration, and premium must be paid before coverage can commence.
9. The English language used in this selling kit is merely a translation of the Thai Version.
10. Other terms, conditions and exclusions subject to the original HEALTH CARE INSURANCE POLICY WORDING.
11. The premium has been included Stamp Duty and V.A.T.
